Thank you for your interest in becoming an authorized dealer of Logo Chair products. Our account set up process involves an initial review of our current distribution, your account, and the trading history of your account with other vendors in our market. After you submit your vendor agreement, application, resale certificate, and W9 our new account team will review this information. Upon approval we will then contact you via either email or phone to inform you on pricing and assist you with your opening order. Click on the links below to access the required paper work and forms that you will need to begin the process. Once you have completely filled out all of the forms, please fax the forms to 615-261-2101 (Attn: New Acct Dept.) All of the following forms are required.
